Happy Hearts is a heart health service for eligible adults on Original Medicare. It brings together an upper-arm blood pressure monitor, the Happy Hearts app, and ongoing care between regular doctor visits. Your own doctor stays in charge.
What is Happy Hearts?
Happy Hearts is operated by Happy Heart Care Services, LLC. It takes part in a Medicare program called the ACCESS Model. ACCESS works within Original Medicare; it is not a separate insurance plan, and you keep the Medicare coverage you already have.
Happy Hearts is not Medicare, CMS, or a government agency, and Medicare does not endorse specific ACCESS participants. You can read more about the program on our ACCESS Model explainer and learn how to verify Happy Hearts.
Who is the program for?
Happy Hearts is intended for eligible adults on Original Medicare. Availability varies by location and ACCESS eligibility. The simplest way to find out whether it may be available to you is to check eligibility. A real person will follow up, answer your questions, and explain what is included before anything is decided.
Happy Hearts is not quite ready everywhere yet. Checking eligibility does not commit you to enroll, and there is no cost to ask.
What does Happy Hearts cost?
If you are eligible and enroll with Happy Hearts, there is no cost to you for the program. You do not need a credit card to check eligibility. Happy Hearts explains what is included before you enroll, so you can make an informed choice.
What is included?
When the program is available to you and you enroll, Happy Hearts ships the heart health kit to your home. The kit described on the current Happy Hearts site includes:
- A blood pressure monitor and upper-arm cuff
- A USB-C charging cable
- A plain-language setup guide
- A QR code to download the Happy Hearts app
- A phone number if you would rather get help from a real person
No extra equipment is needed to take a reading.
How does the monitor work?
The monitor works on its own. Place the cuff on your upper arm, press one button, and the reading appears on the monitor screen. The monitor does not need Wi-Fi.
If you want your readings explained in plain language and kept together over time, a one-time Bluetooth connection links the monitor to the Happy Hearts app. The app guides you through the connection with a few taps. After that, readings sync automatically without typing or a paper log. If you prefer not to use the app, the monitor still shows each reading on its own screen.

What does the app do?
The Happy Hearts app shows your readings in clear, everyday language and keeps your history in one place. It can help you see how your blood pressure changes over time and identify questions you may want to discuss with your doctor. The app does not provide a diagnosis.
How does enrollment work?
- Check eligibility. Answer a few quick questions to see whether Happy Hearts may be available to you.
- Ask questions and check eligibility. A real person will help you understand whether the program may be available to you and what is included.
- Decide when you have the details. Checking eligibility comes with no pressure and no obligation.
When Happy Hearts becomes available in your area, the team will explain the next steps before you enroll.
How does Happy Hearts work with my doctor?
Happy Hearts complements, rather than replaces, the care you receive from your own doctor. You can share a summary of your readings with your doctor whenever you choose. Happy Hearts can also send a summary to your care team, helping them see a pattern over time rather than only one office reading.
Your doctor remains responsible for your medical care. Happy Hearts supports the time between regular visits.
How is my information handled?
Your health information is protected and handled under the Happy Hearts privacy practices. You can review the details in the Privacy Notice and the HIPAA Notice of Privacy Practices. The Happy Hearts FAQ also answers common questions about who can see your readings and how to request account deletion.
